Ask job applicants about their general state of health and their sickness record.

Questions about health and sickness must be asked carefully (to comply with employment legislation) but can help identify any future problems that may arise.

It may not be obvious to the candidate that any health issues they have could affect their ability to take on the role.

This information is also helpful to determine whether you will be required to make adjustments to the working environment prior to employment.

This form also helps you ask disabled applicants appropriate questions and gives you the option to send any applicant to a pre-employment medical.
