Paternity leave request form

This paternity self certification form should be used by employees who wish to take paternity leave.

The form asks the employee to confirm that their partner is pregnant or that they are adopting a child. It also asks how much paternity leave they wish to take and when they wish to take it.

Sections are included that explain:

• the notice an employee is required to give their employer if they wish to take paternity leave
• an explanation of the requirements an employee must meet for them to be elligible to exercise their legal right to paternity leave (and any statutory paternity pay - SPP).

The form makes it clear to employees that there is a time limit by which they must take their paternity leave entitlement.
