Notice of grievance appeal form

Sometimes employees are not satisfied with the outcome of a grievance hearing and want to appeal.

Consideration of all the facts will help you, as an employer, to reach a mutually satisfactory conclusion to the matter.

Use this standard form to ask your employees to explain why they are unhappy with your grievance hearing outcome decision. And to establish the grounds on which they are lodging a grievance appeal, including:

• why they still feel aggrieved
• duration and timeframe
• original facts and background information
• additional evidence for consideration
• what outcome they want
