Return to Work Interview Form

This form helps an employer discuss sickness absence with employees without the fear of reprisal.

Whether it's one day, several days or at the end of a period of long-term sick leave, this form is aimed at helping an employer:

  • manage periods of short-term, recurrent or intermittent sickness absence
  • make employee's aware that periods of sick leave will be managed and monitored
  • bring to an employee's attention that excessive sick leave will not be tolerated
  • assess all employees on a uniformed and equal basis
  • record periods of and reasons for sickness absence
  • ensure receipt and recording of self certification forms and medical certificates
  • check individuals are fit to return to work
  • highlight for discussion any work related reasons affecting sickness absence (e.g. stress, harassment, health and safety issues)
  • develope a plan of action with individuals to prevent future sickness absence occurrences
  • log discussions and schedule a follow-up meeting to manage sickness absence
  • establish background evidence in order to manage disciplinary and capability issues
