Medical Report Request Letter and Form

Not sure what to ask a GP when obtaining a medical opinion or the time frame in which they should reply?

Simple, use this letter and form to request a medical opinion of an employee's state of health from a GP, doctor, specialist or medical practitioner, including:

• confirming an employee's medical consent
• likelihood of a return to work in their current or another role
• current state of health and a medical prognosis of their condition
• details of their current role and physical factors of the role
• reasonable adjustments to aid a return to work
• qualification under the Disability Discrimination Act 1995
• confirmation of when you expect the report to be sent to you (in accordance with the Access to Medical Reports Act 1988)
• space for you to add any additional information or questions for the doctor
