Medical Report Consent Form

Once you've highlighted a medical opinion is required, use this letter and form to:

• inform an employee why you are seeking a medical opinion
• ask the employee to confirm their GP, doctor and medical practitioner contact details
• record the employee's consent for you to obtain medical information from their GP, doctor and/or medical practitioner
  
• advise the employee of their rights under the Access to Medical Reports Act 1988

The same letter and form can also be used to approach your own company doctor or an occupational health advisor for their opinion and prognosis.
