Medical Home Visit Letters

Use these two letters to manage periods of long-term sickness absence.

These letters are aimed at helping an employer formally request a meeting with an employee (at their home, at work or at another agreed location) to discuss an employee's continued absence from work, the company's concerns and agree a plan of action to help facilitate an early return to work.

Combined these two letters form part of long-term sickness absence management activities and help to record discussions held and actions agreed, should pro-longed absence result in ill-health or incapability dismissal.

The Meeting Invitation Letter is used to invite an employee to attend a meeting, on-site or at home, to:

  • discuss their continued sick leave and absence from work
  • discuss payment of salary, wage, SSP and company sick pay during absence
  • explore the reasons for their absence and continued sickness
  • outline the company's concerns
  • explore appropriate company assistance to help facilitate a return to work
  • agree a plan of action going forward
  • inform an employee of their rights at the meeting and during absence
  • obtain a medical report (if appropriate)
  • confirm what actions are required by both employer and employee

The Meeting Outcome Letter is used to confirm the outcome of the above meeting, including:

  • confirming periods of and reasons for long-time absence and sick leave
  • confirm payment of salary, wage, SSP and company sick pay
  • outline company concerns and explored assistance available
  • confirm any help which will be provided and any agreed actions
  • specify whether, and when, a medical opinion will be sought
  • what activities will be undertaken by either party and the next course of action
  • confirmation of what, if any, action will result should long-term sickness absence continue
